PERMANENCY SUPERVISORY DIRECTOR

The Permanency Supervisory Director is expected to use time management skills to complete all time sensitive work in a timely fashion.  Implement billing and invoicing consistently throughout the work week. Assist the Adoption Facilitators in securing documents needed for the families who are in the process of pursuing adoption or PLC.


The Permanency Supervisory Director is responsible for ensuring the compliance of records/charts for the purpose of the adoption goal and for the agencies program compliance with external licensing and regulatory agencies.  And for ensuring the agency is compliant with all 3680 codes that apply to the Adoption Program.

DUTIES AND SKILLS WILL INCLUDE

•    Assigning cases, reviewing case activities, determining training and developmental needs.
•    Reviewing SWAN services within 3-7 days of receipt from staff and submitting them to the County or CUA agency promptly.
•    Implement billing and invoicing consistently throughout the work week.
•    Ability to communicate respectfully with all staff and administration.
•    Propensity for self-development, ability to see shortcomings and willingness to see constructive criticism as positive.
•    Ability to support a consistently developing agency culture that supports successful habits, a friendly work place, mind/body health oriented environment, cordiality, and open, direct communication. 
•    Ability to demonstrate exemplary customer skills by returning all voice and emails within 24 hours, responding and or acknowledging client’s needs within 24 hours, and making copies of important documents that families want returned to them within 24 hours of receiving them. 
•    Ability to define problems, assess situations and behaviors and establish facts and draw valid conclusions.
•    Clear and factual communication with staff and administration on the needs, concerns or activities of the program.
•    Communicate with Regional Technical Assistant from SWAN on the needs, concerns or activities of the program.
•    Train employees reviewing worker performances, and initiating corrective action when necessary.
•    Contact with individuals, their families, representatives of private and public welfare agencies, representatives of professional and community groups, and the general public
•    Supervision of Facilitators, Administrative Assistant and other subordinate staff
•    Evaluate social service programs and make recommendations to improve effectiveness of operations
•    Analyze and resolve complex social work situations and make sound recommendations consistent with social work principles and Adoption program policies.
•    Prepare and analyze reports pertaining to the Adoption programs.
•    Assist the CPO in any areas necessary to maintain the Adoption Program.
•    Good working knowledge of the Child Welfare System. Knowledge of and ability to navigate other systems.
•    Working knowledge of the use of computers and pertinent software programs.

Requirements: Master Degree, 5 years experience in some capacity within the Child, Youth and Family Service Industry.


ADMINISTRATIVE ASSISTANT


The administrative assistant works closely with the President, Vice President, Permanency Supervisory Director in ensuring the program and all its components are operating efficiently.


The Administrative assistant must be organized, communicate clearly and concisely with staff and families.  The Administrative assistant must be able to maintain exemplary records of important documents needed, received that ultimately ensure compliance of case charts.

DUTIES
•    Composing letters for staff and administration
•    Assisting adoption facilitators by visiting families within 5 days of the case opening to ensure all clearances are filled out by all family members 18 and over, and by securing the medical providers contact information for all family members.
•    Sending off release of information documents to providers, schools and agencies to secure medical, educational, psychological information for child profiles.
•    Reviewing charts for compliance on monthly basis.
•    Recording the results of chart compliance in whatever manner is agreed upon by the Administration.
•    Keeping a record of what families have turned in, or need to turn in documents for the compliance of Family Profiles.
•    Assisting Permanency Supervisory Director in maintaining a record of when services have been turned in and payment received.

All other duties as deemed necessary by the administration.


Requirements:  Associate Degree, or 3 years in field or associated field.


PERMANENCY COORDINATOR

The Permanency Coordinator is expected to use time management skills to complete all sensitive work in a timely fashion.  Assist the Permanency Contractors in securing documents needed for families who are in the process of adoption or PLC.

The Permanency Coordinator is responsible for the compliance of the records/charts for the purpose of the adoption goal and for the agencies compliance with external licensing and regulatory agencies.

DUTIES AND SKILLS WILL INCLUDE

  • Assigning cases to contractors.
  • Maintaining consistent contact with contractors and keeping a bi weekly schedule of what tasks and work should be completed by specified time frames.
  • Reviewing SWAN services within 3-7 days of receipt from staff and submitting them to the County or CUA agency promptly
  • Report to the Permanency Supervisory Director and CPO about what services are ready to be billed and invoiced.
  • Ability to communicate respectfully with all staff and administration.
  • Propensity for self-development, ability to see shortcomings and willingness to see constructive criticism as positive.
  • Ability to support a consistently developing agency culture that encourages successful habits, a friendly work place, mind/body health oriented environment, cordiality, and open, direct communication.
  •  Ability to demonstrate exemplary customer skills by returning all voice and emails within 24 hours, responding and or acknowledging client's needs within 24 hours, and making copies of important documents that families want returned to them within 24 hours of receiving them.
  • Clear and factual communication with staff and administration on the needs, concerns or activities of the program.
  • Supervision of Contractors and communicating the evaluations of them to the Permanency Supervisory Director
  •  Good working knowledge of the Child Welfare System. Knowledge of and ability to navigate other systems.
  •  Working knowledge of the use of computers and pertinent software programs.
  • Permanency Coordinator carries a caseload of 10 cases and may perform contract work up to 3 cases totaling 13 cases.

    Requirements: Bachelors Degree, 5 years experience in some capacity within the Child, Youth and Family Service Industry.


  • PERMANENCY FACILITATOR

    Documentation
    •    Good writing skills. The ability to complete Child and Family Profiles in a timely fashion.  70% of all services must be turned in within the timeframe allotted by SWAN.
    •    The ability to tend to benchmarks on an ongoing basis.

    Communication Skills

The ability to maintain at the very least, monthly communication with all clients, County and CUA workers.
•    Clear and factual communication with families, team members and colleagues as required by state and local regulations and agency standards.
•    Advocates for the needs of families to the Permanency Supervisory Director and team members responsible to supporting the family to ensure the goal of permanency does not disrupt.
•    Schedules file reviews upon receiving a case.
•    Schedules visits with the family within 3 days of receiving a case.
•    Manages time in order to complete both direct service requirements as well as paperwork.
•    Prepares supervision notes for weekly supervision.

Compliance Adherence

•    Facilitators participate in the appropriate training and maintain the necessary training hours as mandated by local, state and federal regulations.
•    Facilitators maintain all necessary documents for their case charts as mandated by SWAN, local, state and federal regulations.

Services Implemented

•    Child and Family Profiles, Child Preparation Plans, Child Recruitment Services, Finalization and Post Permanency Services.
•    Attends all pertinent meetings, appointments, and hearings on behalf of the families pursuing adoption or PLC.
•    Conducts home inspections and communicates to Permanency Supervisory Director and team members any safety concerns immediately.
•    Creates incident reports within the time frame and in accordance with local, state and federal regulations.

BASIC SKILL SETS

Adoption Facilitators carry a caseload between 16 -18 and if in good standing,  are allowed to perform contract work.

Requirements: Bachelors in psychology or social work, 3 years experience. Must have valid drivers license.  Full and part time position
